Is there parking? How much does it cost?

Parking at the office is free. There is parking behind the office. We recognize
the anxiety and cost to you when you are at an office where you have to pay for
parking. If your medical problem requires a longer visit, you don’t have to worry
about a parking bill that increasing by the minute.
If someone is driving you, you may be dropped off at the front and have your
driver park in the parking lot behind the office.
What do I need to bring with me?
Please bring your glasses, a list of all the medication (including eye drops) that
you are taking, a list of any herbal remedies that you are taking, your latest
medical insurance card and/or billing information, and a pair of sunglasses. If
your eyes are dilated, some people prefer to have a driver. If your insurance has
a co-pay, this is required at the time of your office visit.

What the e-mail policy with your practice?
In accordance with Washington state regulations, we are sorry that we are
unable to accept e-mail without a signed consent permitting e-mail
correspondence with patients. Department of Health, Medical Quality
Assurance Commission, Policy/Procedure number MD2002-04 “Guidelines for
the Appropriate Use of the Internet in Medical Practice” Section Five D states “A
written agreement should be employed documenting patient informed consent
for the use of patient-physician e-mail. The agreement should be discussed
with and signed by the patient and included in the medical record."
What insurance plans do you accept?
We see people with many different kinds of insurance and with no insurance.
The insurance plans accepted are constantly increasing. They include, but are
not limited to United Healthcare, Medicare, Premera Blue Cross, Regence
BlueShield Participating, Regence BlueSheld Point of Service (POS), Regence
BlueShield for PEBB, Regence BlueShield Preferred (PPO), Regence BlueShield
